The United States has finalized a landmark five-year health partnership with Kenya, committing over $1.6 billion in direct aid, marking the first agreement under President Donald J. Trump’s “America First” global health initiative. The deal, signed on December 4, 2025, emphasizes government-to-government collaboration, prioritizing accountability, partner influence, and streamlined distribution over traditional NGO-led programs.

U.S. Secretary of State Marco Rubio highlighted the shift in approach, criticizing the so-called “NGO industrial complex” for creating parallel health systems that often limit host-country input. “We are not going to spend billions of dollars funding the NGO industrial complex while close and important partners like Kenya ... either have no role to play or have very little influence over how healthcare money is being spent,” Rubio said. The agreement allows Kenya to assume significant control over the allocation and implementation of health funds, enhancing transparency and national ownership.

This five-year plan provides resources for essential health commodities, workforce development, and system strengthening, with total U.S. investment expected to reach $1.7 billion by 2031. Kenya, a key U.S. ally in East African counterterrorism operations, now has a central role in shaping health initiatives within its borders, reflecting a broader strategic alignment between the two nations.
